I ahve exactly the same problem as Mr. Collins (05 Oct/10). The form link is not avaialble, the list link is not available. and the 404 Unspecified error continues to pop up. I would like to move forward with this rebate and am thwarted at every turn in retrieving all pertinent information.

Would someone please tell me why a program of this nature EVER end? It makes no sense that it doesn’t still operate.

Come on you guys. The links don’t work and the program doesn’t even exist any more yet you leave the web site up here. You are what is wrong with the internet.
